CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Gilteritinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with moringa and gilteritinib (Xospata). Your body clears gilteritinib partly through a liver enzyme called CYP3A4. In the test tube, moringa has pulled in two different directions: some lab work suggests it slows that enzyme down, while other work suggests it speeds it up. So we can't even say for sure which way it would push gilteritinib levels.
The key thing is that this is only lab and animal evidence, not real-world proof in people. The one human study we have (with a different drug) showed no change at all. Still, gilteritinib is a serious leukemia treatment where even modest shifts could mean more side effects (like fatigue, low blood counts, or muscle aches) or weaker cancer control. Please don't start moringa without first checking with your oncology pharmacist or doctor.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here is the short version on combining moringa with Xospata (gilteritinib). Gilteritinib is moved around the body partly by a transporter called P-glycoprotein. Lab studies suggest moringa leaf extract may block that transporter, which could in theory let a bit more gilteritinib build up in your system. More drug hanging around can mean stronger side effects, things like fatigue, nausea, diarrhea, muscle aches, or changes in your heart rhythm that your care team already watches closely.
I want to be honest about how shaky this is: it has only been seen in test-tube research, never reported in actual patients, so it stays theoretical. Still, since gilteritinib is a serious cancer medicine, please don't add a concentrated moringa supplement without first checking with your oncology pharmacist or doctor.

What Is Gilteritinib Used For?
Brand name: Xospata
Xospata (gilteritinib) is used to treat adults with acute myeloid leukemia (AML) — a fast-growing cancer of the blood and bone marrow — when the cancer has come back after prior treatment (relapsed) or stopped responding to treatment (refractory). It is only used when a specific genetic change called a FLT3 mutation is found in the cancer cells, confirmed by an FDA-approved test.
Xospata is not used for all types of AML — the FLT3 mutation must be present before treatment begins.
