Why this applies to Vincristine Sulfate

Vincristine Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.

Theoretically, moringa might increase or decrease levels of CYP3A4 substrates.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.

Vincristine is a chemotherapy drug your liver clears using an enzyme called CYP3A4. The concern with moringa is that it might nudge that enzyme one way or the other. The honest answer is that we don't really know which way. Some lab studies suggest moringa slows CYP3A4 down (which could let vincristine build up and worsen side effects like nerve tingling, numbness, constipation, or low blood counts), while other lab work suggests it speeds the enzyme up (which could make the chemo less effective).

This is theoretical, based on test-tube data, not real patients on vincristine. Still, because chemo dosing is so precise and the stakes are high, please don't add moringa without first clearing it with your oncology team.

Theoretically, moringa leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Vincristine is a powerful chemotherapy drug, and your body relies on a transporter called P-glycoprotein to help move it out and keep its levels in check. In lab dish studies, moringa leaf extract appears to slow that transporter down. The concern is mostly theoretical: if moringa blocked P-glycoprotein in people the way it does in a test tube, vincristine could build up and hit harder than intended.

With vincristine, more drug exposure can mean worse nerve-related side effects like tingling, numbness, weakness, or constipation, plus lower blood counts. To be clear, this has not been seen in actual patients, and the evidence is only lab-based. Still, because vincristine has so little room for error, please run any moringa supplement past your oncology team or pharmacist first.
