Adagrasib falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing Ostarine with Adagrasib (Krazati), your targeted cancer pill. This isn't about one drug speeding up or slowing down the other. Instead, it's about doubling up on liver stress. Adagrasib can already raise liver enzymes and, in some people, cause more serious liver injury, so your oncology team likely checks your liver bloodwork regularly while you're on it.

Ostarine, a SARM, has its own track record of bumping up liver markers and has been tied to real cases of liver damage. Stacking the two could, in theory, add up to a bigger hit on your liver. This is a theoretical concern based on how each behaves, not proven cases of them together. Still, please loop in your oncologist or pharmacist before using Ostarine.

Adagrasib also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the thing with this pairing: Adagrasib (Krazati), the cancer medicine you're asking about, can block an enzyme in your body called CYP3A4. Ostarine is partly cleared by that same enzyme, so in theory, slowing it down could let a little more Ostarine build up and slightly raise its effects and side effects. On paper that points toward more Ostarine, not less.

In real life, though, this looks very minor. When researchers paired Ostarine with a strong CYP3A4 blocker, the Ostarine levels barely moved. The bigger concern isn't this interaction at all, it's that Ostarine is an unapproved SARM tied to liver injury, and Adagrasib can already stress the liver and the heart. Please loop in your oncology team before combining them.

About Adagrasib

What Is Adagrasib Used For?

Brand name: Krazati

Krazati (adagrasib) is used to treat adults with locally advanced or metastatic non-small cell lung cancer (NSCLC) that carries a specific genetic change called a KRAS G12C mutation. It is used as a single agent for NSCLC after at least one prior treatment, and patients must be confirmed to have this mutation by an FDA-approved test.

Krazati is also used in combination with another drug called cetuximab to treat adults with KRAS G12C-mutated locally advanced or metastatic colorectal cancer (CRC) who have already been treated with specific prior chemotherapy regimens. Both uses were approved under the FDA's accelerated approval program based on tumor response data, and continued approval may depend on results from confirmatory trials.
