Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ostarine interacts with.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the picture with these two together. Larotrectinib (Vitrakvi) can mildly slow down an enzyme called CYP3A4, and Ostarine happens to be partly broken down by that same enzyme. On paper, that means Larotrectinib could let a little more Ostarine build up, which might nudge up its effects and side effects. But this is mostly theoretical. In real testing, even a strong CYP3A4 blocker barely changed Ostarine levels, so Larotrectinib (a much weaker effect on this enzyme) is unlikely to cause a meaningful problem.

The bigger concern, honestly, is Ostarine itself. It's an unapproved SARM that has been tied to liver injury, and Larotrectinib already requires liver monitoring during cancer treatment. Please talk with your oncologist or pharmacist before combining anything with Vitrakvi.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
