Ribociclib Succinate falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the simple version. Both of these can be hard on your liver. Ribociclib (Kisqali) already carries a known risk of raising your liver enzymes, which is why your oncology team checks bloodwork regularly while you're on it. Ostarine, a SARM that isn't FDA-approved, has also been tied to liver enzyme bumps and even a couple of reported cases of serious liver injury.

Put the two together and the worry is simple: you may be stacking two liver stressors, which could mean a higher chance of liver trouble like fatigue, nausea, yellowing of the skin or eyes, or abnormal liver tests. This concern is theoretical and based on how each behaves, not on direct studies of the pair. Still, given your cancer treatment matters so much, please don't add Ostarine. Talk it over with your oncologist or pharmacist first.

Ribociclib Succinate also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the gist for this pairing. Ribociclib (Kisqali) is one of those medicines that can slow down a liver enzyme called CYP3A4. Since Ostarine is partly broken down by that same enzyme, in theory ribociclib could cause a bit more Ostarine to linger in your body, which might nudge up its effects and side effects, like liver strain.

That said, this is mostly a paper concern. In actual studies, even a strong CYP3A4 blocker (itraconazole) barely moved Ostarine levels, so a real-world problem here is unlikely. My bigger worry is Ostarine itself, which is unapproved, banned in sport, and linked to liver injury, something especially risky alongside cancer treatment. Please loop in your oncologist or pharmacist before combining them.

What Is Ribociclib Used For?

Brand name: Kisqali

Kisqali (ribociclib) tablets are used in adults with hormone receptor (HR)-positive, HER2-negative breast cancer. For early breast cancer (stage II or III, at high risk of coming back), Kisqali is given alongside an aromatase inhibitor as adjuvant treatment — meaning treatment after initial surgery to reduce the risk of the cancer returning.

For advanced or metastatic breast cancer (cancer that has spread), Kisqali is used in combination with either an aromatase inhibitor or fulvestrant — as the first hormonal treatment tried, or after the cancer has grown despite prior hormone therapy.
