Why this applies to Acalabrutinib

Acalabrutinib falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Calquence (acalabrutinib) and Puravive. Puravive contains Panax ginseng, which can tinker with an enzyme called CYP3A4. Your body relies heavily on CYP3A4 to clear acalabrutinib, so anything that nudges that enzyme could shift your drug levels. The tricky part is that ginseng has gone both ways in studies, sometimes slowing the enzyme and sometimes speeding it up, so we honestly can't predict the direction here.

If ginseng slowed your clearance, more acalabrutinib could build up, meaning more bruising, bleeding, headaches, or infection risk. If it sped things up, the drug might work less well against your lymphoma or leukemia. With a cancer therapy, neither guess is one to make on your own. Please run Puravive by your oncologist or pharmacist before starting it.

P-Glycoprotein Substrates

Acalabrutinib also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the situation with Puravive and your Calquence (acalabrutinib). Puravive contains quercetin, a plant compound that may block a transporter in your gut called P-glycoprotein. Acalabrutinib is carried by that same transporter, so in theory, blocking it could let a little more of the drug get absorbed into your blood. If levels nudged up, you might notice more of acalabrutinib's usual side effects, like headache, bruising or bleeding, infections, or feeling tired.

That said, this is mostly theoretical. The evidence is weak and mixed, and quercetin has actually lowered some drugs and raised others. Still, since acalabrutinib treats cancer, I'd rather you not gamble. Please check with your oncology team or pharmacist before adding Puravive.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
