Brentuximab Vedotin falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the connection: Puravive contains Panax ginseng, which can nudge the activity of a liver enzyme called CYP3A4. The active piece of brentuximab vedotin (Adcetris), a small drug called MMAE, is partly cleared by that same enzyme. So in theory, ginseng could shift how much active drug stays in your system. If it slows CYP3A4, more MMAE could linger and raise the odds of side effects like numb or tingling hands and feet (neuropathy), low blood counts, infections, fatigue, or nausea. If it speeds the enzyme up instead, the drug could clear faster.

Honestly, the evidence here is weak and the direction isn't even consistent, so this is more of a caution than a real-world worry. Since brentuximab is serious chemotherapy, please run any supplement past your oncology team or pharmacist before starting it.

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
