Dolutegravir falls under P-Glycoprotein Substrates — the group Puravive interacts with.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the short version. Puravive contains quercetin, a plant compound that may gently block a gut transporter called P-glycoprotein. Your HIV medicine, dolutegravir (Tivicay), is carried by that same pump. So in theory, taking the two together could let a bit more dolutegravir get absorbed, possibly nudging its blood levels up. Dolutegravir is not a prodrug, so more absorption would mean a little more drug, not less. At higher levels you might notice things like headache, trouble sleeping, or stomach upset.

I want to be honest though: this is mostly lab-and-theory level evidence, and studies on quercetin have gone both ways. It's rated a minor, possible interaction. Still, because keeping your HIV treatment steady matters, please run Puravive past your pharmacist or doctor before starting it.

About Dolutegravir

What Is Dolutegravir Used For?

Brand name: Tivicay

Tivicay and Tivicay PD are used to treat HIV-1 infection. They are always taken in combination with other antiretroviral medicines — they are not used alone. Both products are approved for adults (whether new to treatment or previously treated) and for children who have not previously been treated with an integrase inhibitor (INSTI).

Tivicay (the standard tablet) has an additional use: it can be combined with rilpivirine as a complete two-drug regimen for adults whose HIV is already well controlled (virologically suppressed) on a stable treatment plan for at least 6 months, with no history of treatment failure or known resistance to either drug.
