Doravirine falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version for doravirine (Pifeltro): your body relies on a liver enzyme called CYP3A4 to clear it from your system. Puravive contains Panax ginseng, which can nudge that same enzyme. The tricky part is that ginseng has shown mixed results in studies, sometimes slowing this enzyme down and other times speeding it up, so we can't predict the exact direction for any one person.

Why I'd be careful: doravirine isn't a prodrug, so if ginseng speeds up its breakdown, your blood levels could dip and your HIV treatment may not work as well. If it slows things down, you might see more side effects like headache, nausea, or trouble sleeping. The evidence here is weak and the risk is rated minor, but with HIV meds I never want to gamble on levels. Please run Puravive past your pharmacist or doctor first.

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used together with other antiretroviral medications to treat HIV-1 infection in adults and in pediatric patients weighing at least 35 kg (about 77 lbs). It is approved for people who have never been on HIV treatment before, as well as for people who are already virologically suppressed — meaning their HIV is currently undetectable — and want to switch to a new stable regimen.

Pifeltro is not used alone; it must always be part of a combination HIV regimen. Patients switching to a Pifeltro-based regimen must have no history of treatment failure and no known resistance to doravirine.
