Eszopiclone falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Lunesta and Puravive. Your sleep medicine is processed by a liver enzyme called CYP3A4, and Puravive contains Panax ginseng, which can nudge that same enzyme. The tricky part is that ginseng has gone both ways in studies. In some cases it seemed to slow the enzyme (which could let eszopiclone build up, leading to extra grogginess, next-morning drowsiness, dizziness, or that odd metallic taste). In others it sped the enzyme up, which might make the sleep aid a bit weaker.

The good news: this is rated a minor interaction with weak, somewhat conflicting evidence, so it's not something to panic over. Eszopiclone isn't a prodrug, so it does work normally on its own. Still, since the direction isn't settled, check with your pharmacist or doctor before combining them.

Eszopiclone tablets — sold under the brand name Lunesta and as generics — are indicated for the treatment of insomnia. In clinical studies, eszopiclone helped people fall asleep more quickly (reduced sleep latency) and stay asleep through the night (improved sleep maintenance).

Clinical trials supporting its effectiveness lasted up to 6 months in adults and up to 2 weeks in elderly patients. Lunesta and eszopiclone generics are approved for this same indication across the available tablet formulations.
