Lapatinib falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the deal with Puravive and your lapatinib (Tykerb). Puravive contains Panax ginseng, and your body relies on a liver enzyme called CYP3A4 to clear lapatinib out of your system. Ginseng can tug on that enzyme, but honestly the research points in two different directions. In some studies it slows the enzyme down (which could let lapatinib build up and raise the odds of side effects like diarrhea, nausea, rash, or liver strain), while in others it speeds it up (which could make lapatinib less effective at fighting the cancer).

The good news is the evidence here is weak and this is rated a minor, possible interaction. Lapatinib isn't a prodrug, so it doesn't need activating. Still, because this is a serious cancer medicine, I'd run any supplement past your oncologist or pharmacist before starting it.

What Is Lapatinib Used For?

Brand name: Tykerb

Lapatinib tablets — sold under the brand name Tykerb — are used to treat advanced or metastatic breast cancer in patients whose tumors overexpress the HER2 protein. They are always used in combination with another medicine, not alone.

Tykerb is approved in combination with capecitabine for HER2-positive advanced or metastatic breast cancer in patients who have already received prior treatment including an anthracycline, a taxane, and trastuzumab, and whose disease has progressed on trastuzumab. Tykerb is also approved in combination with letrozole for postmenopausal women with hormone receptor-positive, HER2-positive metastatic breast cancer when hormonal therapy is appropriate.
