Ondansetron falls under Cytochrome P450 2D6 (Cyp2D6) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the simple version: Puravive contains Panax ginseng, which a few studies suggest may slightly slow down a liver enzyme called CYP2D6. Your ondansetron (Zofran) is partly handled by that enzyme, so in theory ginseng could let a bit more of it linger in your system. Since ondansetron is active as-is (not a prodrug that needs switching on), more lingering drug would mean slightly stronger effects, things like headache, constipation, or feeling tired.

Honestly, I wouldn't lose sleep over this one. The enzyme slowdown measured was only about 6%, the evidence is weak and even conflicting, and the rating is minor. Still, it's worth a quick mention to your pharmacist or doctor, especially if you take ondansetron regularly or have a heart rhythm concern.

Ondansetron also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the short version: Puravive includes Panax ginseng, which can tug on a liver enzyme called CYP3A4. That enzyme is one of several that help your body break down ondansetron (Zofran). The tricky part is that ginseng has shown mixed behavior in studies, sometimes slowing this enzyme and sometimes speeding it up, so we can't say for sure which direction it would push.

The good news is this is rated a minor interaction with weak evidence. Ondansetron also uses backup enzymes, so any single one matters less. In practice, you might not notice anything, but in theory you could see slightly more side effects like headache or constipation, or slightly less nausea control. Concentrated supplement doses are more likely to count than a little food. Please run it by your pharmacist or doctor first.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
