Why this applies to Saquinavir

Saquinavir falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with saquinavir (Invirase) and Puravive. Puravive contains Panax ginseng, and your body relies heavily on a liver enzyme called CYP3A4 to process saquinavir. The tricky part is that ginseng has gone both ways in studies. In some cases it seems to block CYP3A4 (which could let saquinavir build up, raising the odds of nausea, diarrhea, or other side effects), and in others it appears to speed up CYP3A4 (which could lower saquinavir levels and make it work less well against HIV).

Because saquinavir already needs ritonavir to keep its levels steady, you really want consistent drug levels, not a wild card pushing them up or down. The evidence here is weak and inconsistent, so don't panic, but please check with your pharmacist or HIV doctor before adding Puravive.

Organic Anion-Transporting Polypeptide Substrates (Oatp)

Saquinavir also falls under Organic Anion-Transporting Polypeptide Substrates (Oatp).

Puravive contains quercetin. The simultaneous use of quercetin and OATP substrates (the drug in question in this interaction is reported to be an OATP substrate) may theoretically enhance their effects and adverse effects. In vitro studies indicate that quercetin can inhibit the uptake of estrone-3-sulfate and pravastatin, which are transported by OATP 1B1. Clinical research in healthy males demonstrates that co-administration of quercetin and pravastatin increases the area under the curve (AUC) of pravastatin by 24%, prolongs its half-life by 14%, and reduces its apparent clearance by 18% (source: https://pubmed.ncbi.nlm.nih.gov/22114872/). Overall, quercetin may increase the risk of side effects of drugs that are OATP substrates when taken together.

Here's the gist: Puravive contains quercetin, a plant compound that can gently block a transporter called OATP, which helps move certain drugs into your cells and liver. Saquinavir is one of the medicines that uses this transporter. So in theory, taking the two together could let a bit more saquinavir hang around in your blood, which might mean stronger effects and a higher chance of side effects like nausea, diarrhea, stomach upset, or changes in your blood sugar or fats.

I want to be honest about how shaky this is. The evidence here is theoretical and rated minor, based mostly on lab work and a study using a different drug, not saquinavir itself. Saquinavir is not a prodrug, so the direction makes sense: more drug, not less. Still, since this is an important HIV medicine, please run Puravive by your pharmacist or doctor before starting it.

P-Glycoprotein Substrates

Saquinavir also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the short version on combining Puravive with your HIV medicine, saquinavir (Invirase). Puravive contains quercetin, which in lab settings can block a gut transporter called P-glycoprotein. When that pump is slowed, certain drugs can build up a bit in the body. Saquinavir is moved by that same pump, so in theory its levels could rise, which might mean more nausea, diarrhea, or stomach upset.

The reassuring part: in an actual small human study, several days of quercetin did not meaningfully change saquinavir levels. So this is mostly theoretical for your drug, and the concern is minor. Since keeping HIV treatment steady really matters, I'd still run Puravive past your pharmacist or doctor before starting it.

What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
