Puravive contains Panax ginseng. Panax ginseng is thought to have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias. However, research findings on this matter are conflicting. Here's the gist with Ingrezza (valbenazine) and Puravive. One of Puravive's ingredients is Panax ginseng, and ginseng may slightly stretch out the heart's electrical recovery time, what we call the QT interval. Valbenazine can do the same thing on its own. Stack two QT-stretching things together and, in theory, you raise the odds of an abnormal heart rhythm, which could show up as palpitations, dizziness, or fainting.

That said, the evidence here is mixed and on the weaker side. Some short studies saw a small QT change with ginseng, while longer use showed none. This isn't about ginseng changing how your body breaks valbenazine down, just an additive effect. Before adding Puravive, run it by your pharmacist or doctor, especially if you have heart issues or take other QT-affecting meds.

Cytochrome P450 2D6 (Cyp2D6) Substrates

Valbenazine Tosylate also falls under Cytochrome P450 2D6 (Cyp2D6) Substrates.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

Here's the gist for Ingrezza (valbenazine). One of the Puravive ingredients, Panax ginseng, may very slightly slow down an enzyme called CYP2D6, which helps clear part of valbenazine's active form from your body. If that enzyme runs a touch slower, a bit more of the drug could hang around, which in theory might nudge up side effects like sleepiness, dizziness, or a slow or irregular heartbeat (a known concern called QT prolongation).

The reassuring part: the evidence here is weak, the effect seen in studies was tiny (around 6%), and not all research even agrees it happens. So this is a minor, mostly theoretical concern. Still, since valbenazine affects the heart's rhythm, it's worth a quick chat with your pharmacist or doctor before adding Puravive.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the deal with Ingrezza (valbenazine): your liver uses an enzyme called CYP3A4 to help process it. Puravive contains Panax ginseng, which has shown mixed effects on that same enzyme. In some studies it seemed to slow CYP3A4 down, in others it sped it up, so honestly the direction isn't clear.

If ginseng slowed the breakdown, valbenazine could build up a bit, which might mean more drowsiness or a slightly higher chance of irregular heartbeat. If it sped things up, the medicine might work less well for your tardive dyskinesia. The good news is this is rated a minor interaction with weak evidence, so it's more of a heads-up than an alarm. Still, please check with your pharmacist or doctor before adding Puravive.

Valbenazine — sold as Ingrezza and INGREZZA Sprinkle — is approved for two conditions in adults. The first is tardive dyskinesia, a movement disorder that causes uncontrolled, repetitive facial or body movements, often brought on by long-term use of certain psychiatric medications. The second is chorea associated with Huntington's disease, which refers to the involuntary, dance-like jerking movements that are a hallmark of that condition.

Both Ingrezza and INGREZZA Sprinkle are approved for both of these uses.
