Voriconazole falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the gist for voriconazole (Vfend): one of the ingredients in Puravive is quercetin, which can mildly slow down a liver enzyme called CYP2C9. Voriconazole leans partly on that enzyme to be cleared from your body. If it clears a little slower, levels could creep up, and voriconazole is a drug where blood levels really matter.

Higher levels could mean more of its known side effects, like visual changes (blurry vision or seeing brightness), nausea, headache, or liver enzyme bumps. To be fair, this is based on weak, indirect evidence (a small quercetin study using a different drug), so it may not amount to much. Still, with an antifungal this sensitive, please run it by your pharmacist or doctor before combining them.

Voriconazole also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and voriconazole (Vfend). Puravive contains Panax ginseng, and ginseng can nudge a liver enzyme called CYP3A4, one of the routes your body uses to clear voriconazole. The catch is that the research is genuinely mixed: in some studies ginseng seemed to block this enzyme (which could let voriconazole build up), and in others it seemed to speed it up (which could lower your levels). So we honestly can't predict the direction with confidence.

If levels run high, you might see more visual changes, nausea, or liver strain; if they run low, the infection might not be controlled well. Voriconazole is a serious medicine for serious infections, so don't start Puravive without first checking with your doctor or pharmacist.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
