Aspirin, Codeine Phosphate, Phenobarbital falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

This combination product packs two sedating ingredients: codeine (an opioid pain reliever) and phenobarbital (a barbiturate that calms the nervous system). Both already slow your breathing and make you drowsy on their own. Tianeptine, especially at the high doses found in over-the-counter or gas station products, can act like an opioid itself. Stacking it on top of codeine and phenobarbital means these effects can pile up.

What worries me is the realistic outcome: heavy sedation, confusion, very slow or shallow breathing, and in a serious case, respiratory depression. This concern is based on pharmacology and a case report, not large studies, but the risk is real enough that I'd urge you not to mix these. Please talk with your doctor or pharmacist first.

Chronic use of aspirin might increase the levels and clinical effects of tianeptine.
Aspirin is converted to salicylic acid in the body. In vitro research shows that salicylic acid binds to the same protein sites as tianeptine, displacing protein-bound tianeptine and leading to higher levels of tianeptine in the blood.

This combination product contains aspirin, and that's the part that matters here. When you take aspirin regularly, it turns into salicylic acid in your body. Lab studies (test tube only, not people yet) suggest salicylic acid can bump tianeptine off the proteins it normally clings to in your blood, which could leave more free tianeptine floating around and acting more strongly.

The bigger worry is tianeptine itself. At higher doses it behaves like an opioid, and this medicine already contains codeine (an opioid) plus phenobarbital (a sedative). Stacking those together can pile on drowsiness, slowed breathing, and overdose risk. I'd strongly urge you to skip over-the-counter tianeptine and talk with your doctor or pharmacist before mixing anything like this.
