Why this applies to Cisapride

Cisapride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the concern with pairing uva ursi and cisapride. Cisapride is cleared from your body by a liver enzyme called CYP3A4. In lab dish studies, uva ursi seems to slow that same enzyme down. If that happened in real life, cisapride could build up higher than expected in your blood.

That matters because cisapride is a drug we worry about for the heart. Higher levels can cause a dangerous change in heart rhythm (a long QT), which is exactly why it was pulled from regular pharmacy shelves. So even though this is only theoretical and hasn't been shown in people, I'd treat it cautiously. Please don't start uva ursi without first checking with the doctor or pharmacist managing your cisapride.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the honest picture: uva ursi has only been shown to block a transporter called P-glycoprotein in lab dish studies, not in real people. P-glycoprotein helps move certain drugs out of your body. If uva ursi slowed that pump, levels of cisapride could in theory creep up rather than come down.

Why does that matter with cisapride specifically? This drug can affect your heart's rhythm (a change called QT prolongation), which is exactly why it was pulled from regular pharmacy shelves. Higher levels could raise that risk. The evidence here is weak and purely theoretical, so don't panic, but cisapride is a drug that demands caution. Please check with your doctor or pharmacist before pairing them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.
