Why this applies to Paclitaxel

Paclitaxel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the idea behind this pairing. Your liver uses an enzyme called CYP3A4 to help break down paclitaxel (Taxol, Abraxane). In lab dish testing, uva ursi seemed to slow that same enzyme down. If that were to happen in a real person, paclitaxel could clear more slowly and build up, which might mean stronger side effects like low blood counts, infections, nerve tingling or numbness in the hands and feet, fatigue, or hair loss.

The honest part: this is only seen in test-tube studies, not in actual patients, so it's theoretical. Still, chemotherapy is serious business, so don't add uva ursi (or stop anything) on your own. Please run it by your oncologist or pharmacist first.

Paclitaxel also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the basic idea with uva ursi and your paclitaxel (Taxol or Abraxane). Paclitaxel is partly moved around and pushed out of cells by a transporter called P-glycoprotein. In a lab dish, uva ursi seemed to block that transporter. If that blocking happened in the body, a bit more paclitaxel could hang around, which in theory might mean stronger effects, like more numbness or tingling in the hands and feet, low blood counts, or fatigue.

That said, this is only a theoretical concern seen in test tubes, never confirmed in people, and it's rated minor. Paclitaxel is not a prodrug, so it doesn't get flipped. Still, since you're on chemo, please run any uva ursi or new supplement by your oncology team or pharmacist before starting it.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Paclitaxel (with albumin) Injection Used For?

Brand names: Taxol, Abraxane

Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound) is used to treat three types of cancer. It treats metastatic breast cancer in patients whose prior chemotherapy has stopped working or who relapsed within 6 months of completing adjuvant (post-surgery) chemotherapy. It is also used as a first-line treatment for locally advanced or metastatic non-small cell lung cancer (NSCLC) — given together with carboplatin — in patients who cannot have surgery or radiation therapy.

Additionally, it is approved as a first-line treatment for metastatic adenocarcinoma of the pancreas, given in combination with gemcitabine. All three uses apply specifically to Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound), including the brands Abraxane and related albumin-bound formulations.
