Why this applies to Remdesivir

Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the short version: lab studies suggest uva ursi might slow down an enzyme in your body called CYP3A4, which is one of the pathways that helps clear remdesivir (Veklury). If that enzyme works more slowly, remdesivir could in theory linger a bit longer and build up, which might mean a little more chance of side effects like nausea or changes in liver enzymes.

The good news is this is mostly theoretical. It has only been seen in test-tube studies, not in real people, so we honestly don't know if it matters in everyday life. On top of that, remdesivir is given in the hospital or clinic as an IV by your care team. So bring up any supplements, including uva ursi, with your doctor or pharmacist, and let them guide the timing.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the short version: this is a very theoretical concern, and it likely won't apply to most people. In lab dishes, uva ursi seems to block a transporter called P-glycoprotein, which helps move certain drugs around and out of the body. Remdesivir is a substrate of that transporter, so on paper, blocking it could let a little more remdesivir hang around than expected.

In reality, this has only been seen in test-tube studies, never in actual people, so I wouldn't lose sleep over it. It's also worth knowing that remdesivir (Veklury) is given as an IV in a hospital or clinic, so your care team is already watching you closely. Still, always tell them about any herbs you take, including uva ursi, so they can keep an eye out for things like nausea or liver changes.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
