Neem by Organic India contains 3 active ingredients, all neem. Neem is a traditional leaf extract used to support oral health and skin conditions.
The product also contains pullulan as an inactive ingredient (the capsule material).
Does it work?
Moderate evidence
For gum inflammation (gingivitis) and dental plaque, neem is rated possibly effective based on available research. It's also possibly effective for lice.
For acne and tungiasis (a parasitic skin infection), the evidence isn't established in the data we hold — there isn't enough reliable research to rate it one way or the other.
How safe is it?
Well-documented data
Neem leaf preparations appear generally well tolerated in adults when taken by mouth short-term, though high-quality safety studies haven't been done. The most common side effect when applied to skin is contact dermatitis in sensitive people.
Serious side effects are rare but have been reported with oral neem — these include heart rhythm problems and kidney damage in adults, and severe effects in children given neem oil. Because neem may affect fertility, it's considered likely unsafe in pregnancy and should be avoided.
There isn't enough safety information for breastfeeding, so it's best avoided while nursing.

Neem may raise levels of these drugs and cause unwanted effects.

Through its neem content, this product may increase levels of several types of drugs that your liver processes, potentially making them stronger than intended. The most serious concern is with drugs broken down by a liver enzyme called CYP3A4 (cytochrome P450 3A4 substrates) — these levels could rise and cause unwanted effects — though these interactions have only been seen in lab studies so far, not yet reported in people taking neem.

Neem may also interact with drugs processed by other liver pathways (CYP1A2, CYP2C8, and CYP2C9 substrates) and drugs transported by a kidney protein called P-glycoprotein, raising their levels and effects in similar ways. It can also lower blood sugar when taken with diabetes medications (antidiabetes drugs), raising the risk of low blood sugar (hypoglycemia).
Additionally, neem may weaken immunosuppressants — drugs taken after organ transplants or for autoimmune conditions — by stimulating your immune system.

Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C8 (Cyp2C8) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.

Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.

Immunosuppressants
Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
P-Glycoprotein Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
