Here's the connection worth knowing about. Clarithromycin is an antibiotic that can, on its own, stretch out part of your heart's electrical rhythm (something doctors call the QT interval). Bitter orange contains synephrine, a stimulant that can nudge up your heart rate and blood pressure and may also affect that same rhythm. Stack the two together and, in theory, you could add to the risk of an irregular or racing heartbeat.

I want to be honest about how strong this evidence is: it's mostly theoretical, leaning on a single case report where bitter orange was combined with caffeine. So this isn't a guaranteed problem, but if you feel a pounding heart, dizziness, or fainting, take it seriously. Please check with your pharmacist or doctor before pairing these, especially if you have any heart or blood pressure concerns.

Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Bitter orange might increase levels of drugs metabolized by CYP3A4.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the gist for clarithromycin (Biaxin): bitter orange can mildly block CYP3A4, the gut enzyme that helps process this antibiotic. If that enzyme is slowed, a bit more clarithromycin could stay in your system. That might nudge up its usual side effects, things like an upset stomach, nausea, diarrhea, or a strange metallic taste in your mouth. Clarithromycin can also affect heart rhythm in some people, and since bitter orange is a stimulant that raises heart rate and blood pressure on its own, I'd be a little cautious about stacking the two.

Honestly, the evidence here is limited and comes from small studies using freshly squeezed juice, so the real-world effect is uncertain. Concentrated weight-loss supplements may behave differently than food amounts. Just run it by your pharmacist or doctor before combining them.

Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. Evidence that it works for weight loss or performance is limited, and because it can raise heart rate and blood pressure, people with heart problems, high blood pressure, or who are pregnant should avoid it. Talk to your pharmacist or doctor before using it, especially if you take other stimulants.

About Clarithromycin

What Is Clarithromycin Used For?

Brand names: Biaxin, Biaxin XL

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.

Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
