Rabeprazole Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the situation with cascara sagrada and your rabeprazole (Aciphex). In some lab dish studies, cascara appeared to mildly speed up an enzyme called CYP3A4, which is one of the pathways your body uses to clear rabeprazole. If that happened in people, your stomach acid medicine could be broken down a little faster, which in theory might make it work less well. That could mean heartburn or reflux symptoms creeping back.

The honest part: this is based only on test-tube research, the effect was weak (much weaker than a true enzyme booster like rifampin), and it's rated minor. I wouldn't lose sleep over it, especially with short-term cascara use. If your reflux symptoms return, just mention it to your pharmacist or doctor.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Rabeprazole Used For?

Brand names: Aciphex, Aciphex Sprinkle

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
