Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

This cold and flu combo includes dextromethorphan, the cough suppressant, which your body clears using an enzyme called CYP2D6. In lab studies (not in real people yet), plumbagin from Ceylon Leadwort appears to slow that same enzyme down. In theory, that could let dextromethorphan build up a bit more than usual, which might mean more drowsiness, dizziness, or in larger amounts confusion or a jittery, overstimulated feeling.

I want to keep this in perspective: this is based only on test-tube research, so we don't know if it actually happens in humans. The acetaminophen and chlorpheniramine pieces aren't the main concern here. Still, if you're thinking about combining them, it's worth a quick chat with your pharmacist or doctor first.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the situation with this combo cold medicine. The lab concern is that plumbagin, a compound in Ceylon Leadwort, may slow down a liver enzyme called CYP1A2. Of the three ingredients in Coricidin II, dextromethorphan (the cough suppressant) is the one most likely affected, so in theory its levels could creep up. That might mean more drowsiness, dizziness, or feeling foggy than usual.

Honestly though, this is based only on test-tube data, not real people, so the risk is very uncertain and probably small. I wouldn't lose sleep over it, but since both can cause sleepiness, it's worth a quick chat with your pharmacist or doctor before pairing them, especially if you take Ceylon Leadwort in concentrated supplement form.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Some drugs metabolized by CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), methoxyflurane (Penthrane).

Here's the piece that matters in this combo cold medicine: the acetaminophen (the pain and fever reducer, also known as Tylenol). Acetaminophen is partly handled by a liver enzyme called CYP2E1. In lab dish studies, plumbagin from Ceylon Leadwort can slow that enzyme down. In theory, that could let a bit more acetaminophen linger in your system. Since this is only test-tube data and has never been seen in real people, I wouldn't lose sleep over it.

That said, acetaminophen is hard on the liver in high amounts, so I'd avoid stacking concentrated Ceylon Leadwort supplements with it and never exceed the dose on the box. If you take it regularly or drink alcohol, check with your pharmacist first.
