Here's the piece that matters in this cold-and-cough combo: the dextromethorphan (the cough suppressant) is broken down by a liver enzyme called CYP2D6. In lab dishes, plumbagin from Ceylon Leadwort slows that enzyme down. If the same thing happened in your body, dextromethorphan could build up and lead to more drowsiness, dizziness, or feeling foggy. The other ingredients (acetaminophen, chlorpheniramine, pseudoephedrine) aren't really driven by CYP2D6, so they're less of a concern here.
I want to be honest, though: this is only test-tube data, and it has never been reported in real people. So I wouldn't lose sleep over it. Just mention it to your pharmacist if you plan to use the supplement regularly alongside this medicine.

Here's the honest picture for this cold-and-cough combo. The concern comes from plumbagin in Ceylon Leadwort slowing down a liver enzyme called CYP1A2. The catch is that this combo product (acetaminophen, chlorpheniramine, dextromethorphan, and pseudoephedrine) isn't really cleared mainly by CYP1A2, so this particular interaction is a weak fit. On top of that, the evidence is only from lab dish studies, with nothing reported in actual people.
In plain terms, I wouldn't expect a meaningful problem here. If anything ever did stack up, you might notice more drowsiness or dry mouth from the antihistamine, or feeling jittery from the decongestant. Since the plant itself can be irritating and isn't well studied, please run it by your pharmacist or doctor before combining the two.

This combo cold product packs four ingredients, and one of them, dextromethorphan (the cough suppressant), is partly broken down by a liver enzyme called CYP3A4. In lab dish studies, the plumbagin in Ceylon Leadwort slowed this enzyme down. If that held true in people, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy. The acetaminophen, chlorpheniramine, and pseudoephedrine parts aren't really driven by this enzyme, so they're less of a concern here.
The big caveat: this is only test-tube data, never reported in actual patients, so I wouldn't lose sleep over it. Still, since these are short-term cold meds, it's worth a quick chat with your pharmacist before pairing them with this supplement.

The piece of this cold-and-cough combo that matters here is the acetaminophen (Tylenol). A small amount of acetaminophen is handled by a liver enzyme called CYP2E1. In lab dish studies, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. In theory, that could shift how acetaminophen is processed in the liver.
Here's the honest part: this is only seen in test-tube and animal research, never reported in real people, so I wouldn't lose sleep over it. Still, since acetaminophen is already hard on the liver in higher doses, I'd avoid stacking concentrated Ceylon Leadwort supplements with it. Watch your total daily acetaminophen, skip alcohol, and check with your pharmacist or doctor before combining them.
