Acetaminophen, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

This combo product has two parts. The dextromethorphan piece is the one that matters here. Your body relies on an enzyme called CYP2D6 to break dextromethorphan down. A compound in Ceylon Leadwort, plumbagin, has been shown in lab tests only to slow that enzyme. If that happened in real life, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy or off-balance.
The acetaminophen (Tylenol) part doesn't really go through CYP2D6, so it's largely unaffected. Honestly, this is theoretical and hasn't been seen in people, so I wouldn't lose sleep over it. Still, since both this supplement and cough remedies are short-term things, run it by your pharmacist or doctor first to be safe.

Here's the gist for your Tylenol Cough product. Ceylon Leadwort contains plumbagin, which in test-tube studies slowed down a liver enzyme called CYP1A2. One of the ingredients in your medicine, dextromethorphan (the cough suppressant), is partly handled by that enzyme. So in theory, the supplement could let dextromethorphan build up a bit, which might make you feel more drowsy, dizzy, or lightheaded than usual.
I want to be honest about how thin this evidence is. This has only been seen in the lab, never confirmed in real people, so I wouldn't lose sleep over it. Here's the situation with your Tylenol Cough product, which pairs acetaminophen with dextromethorphan. In lab dishes, the plumbagin in Ceylon Leadwort slowed down an enzyme called CYP3A4. Dextromethorphan is partly handled by that enzyme, so in theory the combo could let a bit more of it linger in your body, nudging up effects like drowsiness, dizziness, or feeling foggy or off-balance.
I want to be straight with you though: this is only test-tube research, with nothing reported in real people, so the concern is fairly soft. Acetaminophen isn't cleared mainly by this pathway either. Here's the gist for your Tylenol Cough product. Part of how your body handles the acetaminophen (the pain and fever reliever) runs through a liver enzyme called CYP2E1. A compound in Ceylon Leadwort, plumbagin, has been shown to slow that enzyme down, but only in lab dish studies, not in real people. In theory, blocking CYP2E1 could change how acetaminophen is processed in the liver.
What's reassuring is how weak this evidence is. There are no human reports of a problem, so for most folks taking normal doses this is unlikely to cause trouble. Still, since acetaminophen can stress the liver if overdone, I'd avoid pairing it with concentrated Leadwort supplements and check with your pharmacist first.
