Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

This combo cold medicine has one ingredient worth flagging here: dextromethorphan, the cough suppressant. Your body relies on an enzyme called CYP2D6 to clear it. A compound in Ceylon Leadwort, plumbagin, has been shown in lab dishes to slow that same enzyme down. In theory, that could let dextromethorphan build up a bit, leading to more dizziness, drowsiness, or a foggy, lightheaded feeling.

I want to be honest about how thin this evidence is. It comes only from test-tube studies and has never been reported in real people, so the risk is largely theoretical. Still, since NyQuil already makes you sleepy (thanks to the doxylamine too), it is worth a quick chat with your pharmacist before pairing them.

Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

If that happened in real people, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or a foggy, lightheaded feeling on top of what the doxylamine antihistamine already causes.

The key thing to remember is this is theoretical, based on test-tube work, not actual human reports. So I wouldn't lose sleep over it, but since NyQuil already makes you sleepy, it's worth a quick chat with your pharmacist before pairing them, especially if you take Ceylon Leadwort regularly.

Here's the piece that matters with NyQuil: it contains acetaminophen (Tylenol), and your liver uses an enzyme called CYP2E1 to handle part of that acetaminophen. In lab dishes, plumbagin from Ceylon Leadwort appears to slow that enzyme down. In plain terms, slowing CYP2E1 could shift how acetaminophen is processed. The good news is that the CYP2E1 pathway is actually the one that creates a toxic liver byproduct, so blocking it isn't necessarily harmful, but we just don't have human data to say for sure.

This is theoretical and based only on test-tube work, so I wouldn't lose sleep over it. Still, since acetaminophen is hard on the liver in large or frequent doses, it's worth a quick chat with your pharmacist before pairing the two.
