Acetaminophen, Doxylamine, Pseudoephedrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist with this combination cold-and-sinus product (acetaminophen, doxylamine, and pseudoephedrine). A compound in Ceylon Leadwort called plumbagin has been shown, only in lab dish studies, to slow down a liver enzyme called CYP1A2. In theory, slowing that enzyme could let certain drugs build up a bit higher than usual.

The honest part: this hasn't been seen in actual people, and none of the three ingredients here lean heavily on CYP1A2 for their breakdown, so a meaningful interaction is unlikely. If you did notice anything, it might look like more of the usual effects, such as extra drowsiness from the doxylamine or feeling jittery, racing heart, or trouble sleeping from the pseudoephedrine. To be safe, run it by your pharmacist or doctor before pairing them.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2E1 (CYP2E1) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2E1. Some drugs metabolized by CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), methoxyflurane (Penthrane).

The piece of this combo product that matters here is the acetaminophen (the Tylenol part). Your liver uses an enzyme called CYP2E1 to handle some of that acetaminophen. In lab dish studies only, a compound in Ceylon Leadwort called plumbagin appears to slow that enzyme down. In theory, slowing it could let a bit more acetaminophen linger in your system. The honest truth, though, is that this has never been shown in real people, so it stays a 'what if' for now.

Here is the practical side. Acetaminophen at normal doses is usually fine, but the liver is its weak spot, especially if you also drink alcohol or take more than directed. I would just keep your total daily acetaminophen sensible and run the supplement by your pharmacist or doctor first.
