Acetaminophen, Phenylephrine, Chlorpheniramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the honest picture with Super Cold Tabs (acetaminophen, phenylephrine, and chlorpheniramine) and Ceylon Leadwort. The concern is plumbagin, a compound in this plant, which slowed down a liver enzyme called CYP1A2 in lab dish studies. If that enzyme is blocked, drugs it handles could build up a bit. But this has only been seen in test tubes, never reported in actual people, so it's very much a theoretical worry right now.

For this cold tablet, none of the three ingredients lean heavily on CYP1A2, so a big interaction is unlikely. Still, if you ever felt extra drowsy, dizzy, or had a racing heartbeat from the phenylephrine, that's worth noting. I'd just run it past your pharmacist before pairing them, especially with concentrated supplement doses.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist with your Super Cold Tabs combo. Ceylon Leadwort contains plumbagin, which in lab dishes can slow down CYP3A4, one of the liver's main drug-clearing enzymes. Of the three ingredients in this product, chlorpheniramine (the antihistamine) is the one partly handled by CYP3A4. If that pathway is blocked, levels could creep up a bit, which might mean more drowsiness, dizziness, dry mouth, or grogginess than usual.

That said, this is purely a theoretical concern based only on test-tube data, not real people, so I wouldn't lose sleep over it. Concentrated supplement doses are more of a question than tiny culinary amounts. Still, before pairing the two, run it by your pharmacist or doctor so they can look at your full picture.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2E1 (CYP2E1) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2E1. Some drugs metabolized by CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), methoxyflurane (Penthrane).

The piece of your cold tablet that matters here is the acetaminophen (the Tylenol component). One liver enzyme called CYP2E1 helps break it down, and in lab dish studies a compound in Ceylon Leadwort (plumbagin) has been shown to slow that enzyme down. In theory, if that enzyme works slower, acetaminophen could linger a bit longer than usual.

I want to be honest with you though: this has only been seen in test tubes and animals, never reported in real people, so it's mostly a 'what if' at this point. The phenylephrine and chlorpheniramine pieces aren't really involved. The bigger everyday concern with acetaminophen is simply not stacking too many products that contain it. If you use Ceylon Leadwort regularly, just mention it to your pharmacist so they can keep an eye on things.
