Here's the honest picture with this combo product (Actifed Plus ES). The concern on file is that plumbagin, a compound in Ceylon Leadwort, can slow down a liver enzyme called CYP1A2. If that enzyme is busy, certain drugs it normally clears could build up a bit. In theory, that might nudge levels of these ingredients higher, leading to more of their usual effects, like a racing or pounding heart and trouble sleeping from the pseudoephedrine, or extra drowsiness and dry mouth from the triprolidine.
That said, this is only a lab finding, and it hasn't actually been seen in real people. None of these three ingredients lean heavily on CYP1A2, so I wouldn't lose sleep over it. Still, run it by your pharmacist before pairing them, especially if you have heart issues or high blood pressure.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the part that matters for Actifed Plus ES: the acetaminophen (Tylenol) piece is the one that overlaps with this supplement. A compound in Ceylon leadwort called plumbagin has slowed down a liver enzyme, CYP2E1, in lab dishes. That enzyme normally turns a small slice of acetaminophen into a toxic byproduct. By blocking it, plumbagin would, in theory, mean less of that harmful byproduct, not more, so this isn't a reason to worry about extra liver harm at normal doses.
The honest bottom line: this is test-tube data only and has never been seen in real people, so it's pretty theoretical. The pseudoephedrine and triprolidine parts aren't really affected here. Still, since acetaminophen and the liver are involved, it's worth a quick word with your pharmacist before pairing them, especially if you drink alcohol or take other acetaminophen products.
