Why this applies to Alpelisib

Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Alpelisib (Piqray): your liver uses an enzyme called CYP3A4 to help clear this drug from your body. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies to slow that enzyme down. If that held true in people, your body might clear Alpelisib more slowly, letting levels creep up and possibly making side effects stronger, things like high blood sugar, diarrhea, rash, nausea, and mouth sores.

I want to be honest about the evidence, though. This is only seen in test-tube and animal research, not in real patients, so it's theoretical for now. Still, since Alpelisib is a serious cancer medicine, please run any Ceylon Leadwort use past your oncologist or pharmacist first.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Alpelisib Used For?

Brand name: Piqray

Piqray is used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and PIK3CA-mutated. It is for use after the cancer has progressed on or after an endocrine-based (hormone-blocking) treatment. An FDA-approved test must detect the mutation.

Vijoice is used for adults and children 2 years and older with severe PROS who need systemic (whole-body) therapy. This use has accelerated approval, which is based on response rate and duration of response. Continued approval may depend on confirmatory trials.
