Why this applies to Anagrelide

Anagrelide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for Anagrelide (Agrylin): your body relies on a liver enzyme called CYP1A2 to help break this medicine down. A compound in Ceylon Leadwort called plumbagin can slow that same enzyme. If clearance slows, anagrelide could build up a bit more than expected, which may amplify its usual effects like headache, a racing or pounding heartbeat (palpitations), dizziness, fluid retention, or a bigger drop in your platelet count.

Now the reassuring part: this is only seen in lab dishes so far, never confirmed in real people, so it's theoretical. I wouldn't panic, but since anagrelide affects your heart and blood counts, it's worth a quick chat with your pharmacist or doctor before adding this supplement.

Anagrelide also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the simple version: anagrelide already lowers your platelet count and slightly reduces how well your blood clots. In animal studies, Ceylon Leadwort root extract made platelets stick together less and made bleeding last longer. Stack that on top of anagrelide, and the theoretical worry is a bit more bleeding risk, things like easy bruising, nosebleeds, bleeding gums, or cuts that are slow to stop.

The important caveat: this comes from lab and animal research only, not human studies, so we honestly don't know how big a deal it is in people. It's a reasonable thing to be cautious about, not something to panic over. Please don't change anything on your own. Just loop in your pharmacist or doctor before adding this supplement so they can keep an eye on your platelets and any bleeding.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Anagrelide Used For?

Brand name: Agrylin

Anagrelide — sold as Agrylin and as generic anagrelide capsules — is used to treat thrombocythemia secondary to myeloproliferative neoplasms. That means it's prescribed when a bone marrow disorder causes the body to produce too many platelets, raising the risk of dangerous blood clots.

By bringing platelet counts down to a safer range, anagrelide also helps reduce related symptoms, including bleeding and clotting events (thrombo-hemorrhagic events).
