Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down and clear brexpiprazole (Rexulti). A compound in Ceylon Leadwort called plumbagin has been shown to slow that same enzyme, but so far only in lab dish (in vitro) studies, not in real people. This is a theoretical concern, not something that has actually been reported in patients.

If it did happen, the worry would be that brexpiprazole clears more slowly and builds up, which could mean more side effects like drowsiness, restlessness, dizziness, weight gain, or feeling shaky. Since brexpiprazole isn't a prodrug, slowing the enzyme means a stronger effect, not a weaker one. Before adding this supplement, please check with your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help break down brexpiprazole (Rexulti). Ceylon Leadwort contains a compound, plumbagin, that has slowed this same enzyme down in lab dish studies. If that were to happen in real life, brexpiprazole could clear out of your system more slowly and build up to higher levels than expected.

In practice, that might mean more of this drug's usual effects, like drowsiness, restlessness, dizziness, or weight changes. I want to be honest, though: this is only seen in test-tube research, not in actual people, so it's theoretical for now. Still, since brexpiprazole works on your brain chemistry, it's worth running it past your pharmacist or doctor before combining the two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
