Buproprion, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Auvelity is an interesting combo. The bupropion part actually works by blocking CYP2D6, which on purpose slows down how your body clears the dextromethorphan so it can build up and do its job in the brain. Plumbagin, a compound in Ceylon Leadwort, has also blocked CYP2D6 in lab dishes. In theory, stacking another blocker on top could push dextromethorphan levels even higher, which might mean more dizziness, drowsiness, nausea, or feeling foggy.

I'll be honest though, this is only test-tube data and hasn't been seen in real people, so I don't want to alarm you. Still, since this medication is finely balanced, it's worth a quick chat with your pharmacist or doctor before adding the supplement.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Auvelity combines bupropion and dextromethorphan, and the bupropion part is included on purpose to slow how fast your body clears the dextromethorphan, which boosts its brain effect. Ceylon Leadwort's compound plumbagin has been shown in lab dishes to block the CYP3A4 enzyme. CYP3A4 plays a role in handling parts of this combo, so in theory the supplement could let levels drift higher than expected.

If that happened, you might notice more dizziness, drowsiness, headache, jitteriness, dry mouth, or trouble sleeping. Keep in mind this is only a test-tube finding, not something reported in real patients, so I wouldn't panic. Still, since bupropion lowers the seizure threshold, it's worth checking with your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2B6 (CYP2B6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).

Auvelity combines bupropion and dextromethorphan. Here, the bupropion piece is the one to watch. Your liver uses an enzyme called CYP2B6 to break bupropion down, and lab studies suggest plumbagin (a compound in Ceylon Leadwort) can block that enzyme. If that slowdown happened in real life, bupropion could build up a bit more than expected. That might mean more of its typical side effects, like trouble sleeping, jitteriness, a faster heartbeat, dry mouth, or, at higher levels, a lower seizure threshold.

I want to be honest though: this is only seen in a test tube, not in people, so it stays theoretical for now. Still, before pairing this herb with Auvelity, please run it by your pharmacist or doctor first.
