Chlorpheniramine, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with this combo product (Tussend). The hydrocodone part is actually a prodrug that your body partly turns into a stronger painkiller using the liver enzyme CYP2D6. In test-tube studies, the plumbagin in Ceylon Leadwort can block that same enzyme. If that held true in people, you might get less pain relief from the hydrocodone, since less of it gets activated.

The chlorpheniramine (antihistamine) side could lean the other way and build up a bit, possibly meaning more drowsiness or dry mouth. Honestly, though, this is only lab data, not seen in real patients, so I wouldn't lose sleep over it. Still, run it past your pharmacist or doctor before pairing them.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

The hydrocodone part of Tussend is the piece I'd watch here. Hydrocodone is partly handled by an enzyme called CYP3A4, and lab studies suggest plumbagin (a compound in Ceylon Leadwort) can slow that enzyme down. In theory, that could let hydrocodone build up a bit, which might mean more drowsiness, dizziness, constipation, or slowed breathing in higher doses.

The important word here is theoretical. This has only been seen in test-tube research, never reported in real people, so the evidence is weak. I'm not telling you to avoid anything, but since this product also contains an opioid, please run it by your pharmacist or doctor before combining them.

Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
