Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Here's the gist with this combo cough and cold medicine. Both ingredients lean on a liver enzyme called CYP2D6 to be cleared out, especially the dextromethorphan side. Plumbagin, a compound in Ceylon Leadwort, has been shown to slow that same enzyme down, but only in lab dish studies so far. In theory, that could let these drugs build up a bit more than usual.
If that happened, you might feel extra drowsy, foggy, or dizzy, with a dry mouth from the chlorpheniramine, and at higher dextromethorphan levels some people get restless or queasy. To be clear, this has never actually been reported in real people, so I wouldn't panic. Still, it's worth a quick chat with your pharmacist before pairing them.

This combo cough and cold product pairs an antihistamine (chlorpheniramine) with a cough suppressant (dextromethorphan). Both are partly broken down by a liver enzyme called CYP3A4. In lab dish studies only, plumbagin from Ceylon Leadwort appears to slow that enzyme down. If that held true in people, your body might clear these medicines a little more slowly, letting their levels creep up.
What that could feel like: more drowsiness, dizziness, dry mouth, or feeling foggy, and with extra dextromethorphan, possible nausea or a spaced-out feeling. Keep in mind this is theoretical, shown only in test tubes, never reported in real patients. Still, since both cause sleepiness, it is worth a quick chat with your pharmacist before combining them.

Here's the honest take on this one. The concern on file is that plumbagin, a compound in Ceylon Leadwort, can block a liver enzyme called CYP2E1 in lab dishes. But your cough and cold combo, chlorpheniramine and dextromethorphan, isn't really cleared by CYP2E1 in any meaningful way. Dextromethorphan mainly relies on CYP2D6, and chlorpheniramine uses other pathways. So the specific enzyme this herb touches doesn't line up well with how your body handles these two drugs.
On top of that, this is test-tube only evidence with nothing reported in real people. I wouldn't lose sleep over it. Still, both ingredients can cause drowsiness and dizziness, so if you ever feel extra groggy, check in with your pharmacist before mixing anything new.
