Clozapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist with clozapine: your body relies on a liver enzyme called CYP2D6 (along with others) to help clear it out. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies to slow that enzyme down. If that happened in a real person, clozapine could build up a bit higher than expected, which might mean more drowsiness, dizziness, drooling, a faster heartbeat, or low blood pressure.

The honest part: this is only theoretical right now, based on test-tube research, not on people taking both together. Clozapine is a tricky medication that already needs close monitoring, so I wouldn't add Ceylon Leadwort without first checking with your doctor or pharmacist.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for Clozapine: your liver relies heavily on an enzyme called CYP1A2 to break this drug down and keep its level in a safe range. The plumbagin in Ceylon Leadwort has been shown to slow that same enzyme in lab tests. If that happened in your body, clozapine could clear more slowly and build up, which may mean stronger sedation, dizziness, drooling, low blood pressure, a faster heartbeat, or in rare cases seizures.

I'll be honest though, this is only a test-tube finding and has never been reported in real people. Clozapine is a tricky medication that already needs careful monitoring, so even a small change matters. Please don't start or stop anything on your own, just check with your pharmacist or doctor first.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
