Why this applies to Codeine Sulfate

Codeine Sulfate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with codeine: it's actually a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves your pain. Ceylon Leadwort contains plumbagin, which in lab tests appears to block that same enzyme.

So in theory, pairing the two could mean less codeine gets converted to morphine, giving you weaker pain relief than you'd expect. The good news is this is only based on test-tube research, not real people, so it's far from certain. If your pain isn't well controlled while taking both, don't adjust anything on your own. Just give your pharmacist or doctor a call so they can sort it out with you.
