Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the honest picture for Vosevi (sofosbuvir, velpatasvir, voxilaprevir). The concern on file is that plumbagin, a compound in Ceylon Leadwort, blocked an enzyme called CYP1A2 in the test tube. In theory, slowing that enzyme could let drugs it handles build up a bit higher than usual. For Vosevi, higher levels might mean a greater chance of side effects like headache, tiredness, nausea, or diarrhea.

But I want to be clear: this is laboratory-only evidence, never actually seen in real people, and CYP1A2 isn't even the main pathway these hepatitis C drugs rely on. So I wouldn't lose sleep over it. Still, because finishing your full hepatitis C course matters so much, please run any Ceylon Leadwort use past your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Vosevi, the three-in-one hepatitis C pill. One ingredient in Ceylon Leadwort, plumbagin, has been shown in test-tube studies to slow down an enzyme called CYP3A4. Two of the three drugs in Vosevi (velpatasvir and voxilaprevir) are partly cleared by that enzyme. If it gets blocked, those levels could theoretically creep up, which might mean more side effects like nausea, headache, or tiredness.

The key word is theoretical. This has only been seen in lab dishes, never in real people, so I wouldn't lose sleep over it. Still, because you want your hepatitis C treatment to work just right, please run any supplement past your pharmacist or doctor before adding it.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2B6 (CYP2B6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).

That said, this is very weak evidence (lab dishes only, never seen in real people), and Vosevi isn't mainly cleared through CYP2B6 anyway. So I wouldn't lose sleep over it. The bigger point is that finishing your full hepatitis C course exactly as prescribed is what cures the infection, and adding an unstudied herb that can irritate the gut isn't worth the gamble. Please check with your pharmacist or doctor before using Ceylon leadwort during treatment.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.

Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
