Why this applies to Propoxyphene

Propoxyphene falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist with propoxyphene (Darvon): your body uses an enzyme called CYP2D6 to help process this opioid pain reliever. In test-tube studies, the plumbagin in Ceylon Leadwort appears to slow that enzyme down. If that happened in real life, propoxyphene could build up a bit more than usual, which might mean stronger effects and a higher chance of side effects like drowsiness, dizziness, nausea, constipation, or slowed breathing.

I'll be honest though, this is only theoretical so far. It's based on lab research, not on real people taking the two together, so we can't say how big a deal it would actually be. Still, since propoxyphene is a sedating opioid, it's worth a quick chat with your pharmacist or doctor before combining them.

This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.

It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
