Diclofenac epolamine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist: your body relies on a liver enzyme called CYP2C9 to break down diclofenac, the active ingredient in Flector and Licart patches. In lab dishes, plumbagin (a compound in Ceylon Leadwort) slowed that same enzyme down. In theory, that could let diclofenac build up a bit higher than usual, which may nudge up the typical NSAID side effects like stomach upset, heartburn, fluid retention, or, with heavier use, kidney strain.

Keep this in perspective. This is only test-tube evidence and hasn't been seen in real people, so I wouldn't lose sleep over it. A topical patch also puts less drug into your bloodstream than pills do. Still, before combining them, check in with your pharmacist or doctor, especially if you have stomach or kidney concerns.

Diclofenac epolamine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing to keep in mind: diclofenac is an NSAID, and like other NSAIDs it can already nudge up your bleeding risk a little by making platelets less sticky. Ceylon Leadwort appears to do something similar. In animal studies, its root extract reduced platelet stickiness and made bleeding times longer. Stack the two together and, in theory, you could see more bruising, nosebleeds, stomach upset, or bleeding that takes longer to stop.

I want to be honest about the evidence, though. This bleeding effect has only shown up in lab and animal research, not in people, so it's more of a caution than a proven problem. Flector and Licart are patches, so less drug gets into your blood than with pills, which lowers the worry a bit. Still, it's worth a quick chat with your pharmacist or doctor before combining them.
