Donepezil falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for donepezil (Aricept): your body partly relies on a liver enzyme called CYP2D6 to clear this medication. A compound in Ceylon Leadwort, plumbagin, has been shown to slow that enzyme down in lab tests. If that same effect happened in people, donepezil could build up a bit more than expected, which might mean stronger cholinergic side effects like nausea, diarrhea, dizziness, muscle cramps, a slow heartbeat, or trouble sleeping.

I want to be honest about how shaky this is though. So far this is only seen in test-tube studies, never confirmed in actual patients, so it's more of a theoretical caution than a proven problem. Still, since donepezil can be sensitive to changes, please run it by your pharmacist or doctor before adding this supplement rather than guessing.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for donepezil (Aricept). Your liver uses an enzyme called CYP3A4 to help break it down and clear it. Ceylon Leadwort contains plumbagin, which in lab studies slowed that same enzyme. If that happened in your body, donepezil could build up a bit higher than usual, which can mean more of its typical side effects, things like nausea, diarrhea, loss of appetite, dizziness, a slower heartbeat, or trouble sleeping.

The important caveat: this is only seen in test tubes so far, not in real people, so it stays theoretical. Still, I'd loop in your pharmacist or doctor before combining them, especially since concentrated supplement doses are stronger than any food amount. Don't stop your donepezil on your own.
