Drospirenone, Estetrol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the situation with Nextstellis (the birth control pill that combines drospirenone and estetrol) and Ceylon Leadwort. A compound in the plant called plumbagin can slow down a liver enzyme known as CYP1A2. If that enzyme runs slower, certain medicines it normally clears can build up a bit in your body.

The honest truth is this is a theoretical concern. It has only been seen in lab dishes, not in real people, and the estrogen part of this pill (estetrol) is not heavily dependent on CYP1A2 anyway. So I wouldn't expect a meaningful change in how your pill works or in side effects like breast tenderness, nausea, or spotting. Still, since the safety of this plant isn't well studied, please run it by your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Nextstellis combines drospirenone and estetrol, two hormones in this birth control pill that your liver partly clears using an enzyme called CYP3A4. A compound in Ceylon Leadwort called plumbagin appears to slow that enzyme down, at least in lab tests. In theory, that could let these hormones build up a bit higher than usual, which might mean more typical pill side effects like breast tenderness, nausea, headache, or mood changes.

Here's the honest part: this is only based on test-tube research, not real people, so we truly don't know if it matters in everyday life. It is not a prodrug, so the direction would be toward stronger, not weaker, hormone effects. Since the supplement itself isn't well studied for safety, I'd run it by your pharmacist or doctor before combining them.

Nextstellis combines drospirenone with estetrol, a plant-derived estrogen, and you're taking it as a birth control pill. The concern here is that, in lab and animal studies, the plumbagin in Ceylon Leadwort showed anti-estrogenic activity, meaning it may work against the estrogen part of your pill. If that effect happened in the body, it could theoretically blunt how well the hormones do their job, which for a contraceptive raises the worry of reduced reliability or breakthrough bleeding and spotting.

I want to be honest: this is based only on test-tube and animal research, not real people, so we truly don't know if it matters at human doses. Still, because this pill is preventing pregnancy, I'd play it safe and check with your pharmacist or doctor before combining them.
