Ceylon Leadwort and Durvalumab

Here is the interaction we found between these two, with the evidence behind it.

Moderate

IMMUNOSUPPRESSANTS

Why this applies to Durvalumab

Durvalumab falls under IMMUNOSUPPRESSANTS — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's an interesting wrinkle with Durvalumab (Imfinzi). Unlike the classic immunosuppressants in that database list, Durvalumab actually works in the opposite direction. It's an immunotherapy drug that revs up your immune system so it can attack cancer cells. So the worry here is a little different than for someone on a transplant drug.

The concern on file is purely theoretical and comes from lab and animal studies only, where plumbagin (a compound in Ceylon Leadwort) calmed the immune system down. In theory, that could work against what Durvalumab is trying to do, possibly blunting its cancer-fighting effect. There's no solid human evidence proving this happens, but since Durvalumab is serious cancer treatment, I wouldn't take chances. Please run any supplement past your oncology team first.

From our pharmacist’s overview on the Durvalumab page

“Nearly everything on this page interacts with durvalumab (Imfinzi) for the same underlying reason, immune stimulation. Durvalumab is a cancer immunotherapy, and because the treatment works through the immune system itself, supplements that push immune activity in their own direction (echinacea, ashwagandha, astragalus, and dozens of si…”
