Why this applies to Fosaprepitant

Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with these two. Fosaprepitant (Emend Injection) is given before chemotherapy to prevent nausea and vomiting. Once it's in your body it converts to aprepitant, and that active form is both broken down by the liver enzyme CYP3A4 and is itself something of a CYP3A4 substrate. Ceylon Leadwort contains plumbagin, which in lab dishes has slowed down that same enzyme.

In theory, if Ceylon Leadwort blocked CYP3A4, aprepitant could clear a bit more slowly and build up, possibly adding to side effects like fatigue, headache, hiccups, or constipation. But honestly, this is only seen in test-tube studies, never reported in real people, so I wouldn't lose sleep over it. Since Emend is usually a short hospital course, just mention any supplements to your care team beforehand.
