Why this applies to Glyburide, Metformin

Glyburide, Metformin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the situation with your Glucovance, which combines glyburide and metformin. The glyburide part is cleared from your body by a liver enzyme called CYP2C9. In lab studies, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. If that happened in real life, glyburide could build up and push your blood sugar too low. Signs of that include shakiness, sweating, dizziness, confusion, or feeling suddenly weak and hungry. Metformin itself isn't handled this way, so the concern is really the glyburide.

The good news: this comes only from test-tube research, not from people, so it's theoretical for now. Still, since low blood sugar is no joke, I'd run it by your pharmacist or doctor before adding this supplement.

Ceylon leadwort root extract has been shown to both increase and decrease blood glucose levels in animal research. This effect has not yet been demonstrated in humans. Theoretically, Ceylon leadwort might reduce or potentiate the effects of antidiabetes drugs. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the honest picture: Glucovance combines glyburide and metformin, two medicines that already lower your blood sugar. Ceylon Leadwort is a bit of a wild card. In animal studies, its root extract has pushed blood sugar in both directions, sometimes lowering it and sometimes raising it. We don't have human studies to tell us which way it leans in real life, so this is a theoretical concern, not a proven one.

The thing to watch for is your sugar swinging too low. Glyburide especially can cause shakiness, sweating, confusion, or dizziness if levels drop too far. If you decide to try this supplement, check your glucose more closely than usual and loop in your pharmacist or doctor first. Don't change your Glucovance dose on your own.
