Isavuconazonium Sulfate (IV) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the short version for Cresemba (isavuconazonium). Your liver clears this antifungal partly through an enzyme called CYP3A4. A compound in Ceylon Leadwort, plumbagin, has been shown in the lab to slow that same enzyme down. If that held true in people, your body might clear the drug a little more slowly, letting levels drift up. With Cresemba that could mean more of its usual side effects, like nausea, headache, low potassium, or elevated liver enzymes.

I want to be honest about the strength of this: it's only been seen in test-tube and animal studies, never confirmed in real patients, so it's more of a theoretical caution than a proven problem. Still, since you're treating a serious fungal infection, I'd loop in your doctor or pharmacist before adding this supplement.

What Is Isavuconazonium Used For?

Brand name: Cresemba

Cresemba (isavuconazonium sulfate) is used to treat two serious, life-threatening fungal infections: invasive aspergillosis and invasive mucormycosis. These infections can spread through the body and are most dangerous in people with weakened immune systems.

Cresemba for injection is approved for adults and children 1 year of age and older. Cresemba capsules are approved for adults and children 6 years of age and older who weigh at least 16 kilograms (about 35 pounds).
