Why this applies to Lomitapide

Lomitapide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the thing to know about Lomitapide (Juxtapid): your liver clears it using an enzyme called CYP3A4. A compound in Ceylon Leadwort called plumbagin has been shown, in lab dishes only, to slow that same enzyme down. If that effect held true in people, your body might clear lomitapide more slowly, letting levels climb higher than intended.

That matters because lomitapide already carries real risks of liver enzyme elevations and fatty liver, plus stomach side effects like diarrhea, nausea, and cramping. Higher levels could make those worse. That said, this is only theoretical right now, based on test-tube studies, with nothing reported in actual patients. Lomitapide is a drug with strict dosing rules, so please don't combine the two without first checking with your pharmacist or doctor.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Lomitapide

What Is Lomitapide Used For?

Brand name: Juxtapid

Juxtapid (lomitapide) is used alongside a low-fat diet and other cholesterol-lowering treatments — including LDL apheresis (a procedure that filters LDL from the blood) where available — to lower LDL cholesterol, total cholesterol, apolipoprotein B, and non-HDL cholesterol in patients with homozygous familial hypercholesterolemia (HoFH). HoFH is a rare, inherited condition that causes extremely high LDL cholesterol levels from birth.

Juxtapid is approved for use in adults and in children aged 2 years and older with HoFH. It has not been proven safe or effective for other types of high cholesterol, including the more common heterozygous familial hypercholesterolemia (HeFH). Its effect on heart attacks, strokes, or other cardiovascular events has not been established.
