Why this applies to Lorlatinib

Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down lorlatinib (Lorbrena) and keep its levels in a safe range. Ceylon Leadwort contains plumbagin, which in lab studies slowed that same enzyme down. If that played out in people, lorlatinib could clear more slowly and build up, which might mean more of its known side effects, things like high cholesterol and triglycerides, swelling, mood or thinking changes, slow heartbeat, and nerve tingling.

I want to be honest about the strength here: this is only a test-tube finding, and it hasn't been seen in actual patients. Still, lorlatinib is a serious cancer medicine, so don't add this supplement on your own. Please check with your oncologist or pharmacist first.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Lorlatinib Used For?

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
