Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Opsynvi combines two medicines for pulmonary hypertension: macitentan and tadalafil. Ceylon Leadwort contains plumbagin, which in test-tube studies slowed down a liver enzyme called CYP2C9. Macitentan is partly cleared by this enzyme, so in theory the supplement could let it build up a bit. If that happened, you might notice more of macitentan's usual effects, like swelling, low blood pressure, headache, stuffy nose, or a drop in red blood cells (anemia).

I want to be honest though: this is only lab-based evidence and hasn't been seen in real people, so the actual risk is uncertain. Still, because these are serious drugs, please check with your pharmacist or doctor before adding Ceylon Leadwort. Don't change your Opsynvi on your own.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Opsynvi combines two medicines, macitentan and tadalafil, and both are broken down in the body partly by an enzyme called CYP3A4. In lab dishes, the plumbagin in Ceylon Leadwort slows this enzyme down. If that same effect happened in people, your body would clear these drugs a bit more slowly, so their levels could creep up. That might mean stronger side effects like headache, flushing, low blood pressure, dizziness, an upset stomach, or nasal congestion.

Here's the honest part: this is only a theoretical concern based on test-tube work, not anything reported in real patients, so I don't want to alarm you. Still, since Opsynvi treats pulmonary hypertension and blood pressure changes matter, please check with your pharmacist or doctor before pairing them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
