Why this applies to Modafinil

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Your body partly relies on an enzyme called CYP3A4 to clear modafinil out of your system. In lab dish studies, plumbagin (the main active compound in Ceylon Leadwort) appears to slow that same enzyme down. If that held true in people, modafinil could linger a bit longer and build up, which might mean stronger or longer wakefulness effects, plus a higher chance of side effects like headache, jitteriness, a racing or pounding heart, anxiety, or trouble sleeping at night.

Here's the reassuring part: this is only a theoretical concern based on test-tube work, and it has not actually been reported in real patients. I wouldn't lose sleep over it, but since both can affect each other, it's worth a quick chat with your pharmacist or doctor before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
